Call our main office at (810) 987-5300 for appointments. For WIC appointments, call (810) 987-8222. Blue Water Area Transit Public transportation system, includes bus routes and information. A direct route to the Main Office, please select Routes 2 and 9. Administration, Emergency Preparedness, Planning and Response, Environmental Health, Health Education and Community Outreach, and Family and Community Health Services which includes: Adult Immunization Travel Program, Childhood Immunizations, Children’s Special Health Care Services, Clinic Services, Communicable Disease Control/Prevention, Hearing and Vision Screening, Lead Testing, Maternal Infant Health (MIHP), and Women, Infant, and Children’s Supplemental Food Program (WIC).

Mailing Address: 3415 28th Street Port Huron, MI 48060 Blue Water Area Transit Public transportation system, includes bus routes and information. A direct route to the Teen Health Center, please select Routes 1 and 3. Medical clinic providing acute and chronic health care to adolescents, 10-21 years of age.
