Non-Medical Immunization Exemptions
The online exemption form is intended only for children who attend—or will attend—school or a group program, such as a childcare program, in St. Clair County, Michigan, and whose parent or guardian is a county resident at the time of attendance. A separate exemption form must be completed for each child in a household.
After properly submitting the online form, parents will receive a certified exemption by email within five days.
